Carers

Are you a carer? Would you like this to be noted on your medical record? At Pak Health Centre, we maintain a Carers Register, a dedicated list for patients providing unpaid care and support to a family member or friend with a long-term health condition.

In addition to being registered as a carer, we offer various supportive measures:

  • Flexible Appointment Times: We accommodate your schedule and that of the person you care for.
  • Annual Health Checks: Carers are entitled to annual health check-ups.
  • Carers Board: A display of useful information for carers, including details of the Carers Count service.
  • Carers Champion: An allocated staff member who can provide more information about Carers Count and refer you for support.
  • Information Sharing: We agree to share information about the person you care for, with their consent. If they lack capacity or are unable to consent, we may still share information if you hold Lasting Power of Attorney for Health and Welfare (proof may be required).
  • Free Flu Jab: Registered carers are entitled to a free flu jab annually.
