We value your feedback and strive to continuously improve our services at Pak Health Centre. To better understand your needs and perceptions, we are establishing a virtual patient representation group. This group will provide you with the opportunity to voice your opinions and help us enhance our care.
As a member of this group, you may be asked questions about various aspects of our practice, such as our opening hours or the quality of care you have received. We will contact you via email and keep our surveys brief to minimize the time commitment on your part.
Our goal is to assemble a diverse group of around a hundred patients, representing a wide range of demographics. We are looking for input from young individuals, working professionals, retirees, individuals with long-term conditions, and those from non-British ethnic backgrounds.

Please note that we will not respond to any medical information or questions received through the survey.
